**MS Word fill-in form to report 2012 District Assembly Delegates. The Church Board Secretary and the Local Pastor must sign this form to validate the election of the delegates. No delegates from a local church will be seated without this form being submitted. This form also includes a section to list the ministerial staff of local churches, who by virtue of their assigned status are also ex-officio delegates of the district assembly.
