2010 Certificate of Delegate Election - District Assembly only*	2010 District Assembly Delegates. The Church Board Secretary and the Local Pastor must sign this form to validate the election of the delegates. Delegates from a local church will NOT be seated without this form being submitted. This form also includes a section to list the ministerial staff of local churches. All delegates will be verified before being seated.	PDF Format. Must be signed by the Pastor and Board Secretary
2010 Local Church Deceased Members*	**MS Word fill-in form for District Assembly Memorial Roll (Not used for the NMI Memorial Roll). Please save this form to your computer and open on your computer to fill-in. Do not fill-in from the website. Must be printed and signed after filled-in.	Must be Signed by the Pastor
2010 Pastor's Written Report Outline for the GS*	MS Word Documnet. This is the outline to use to write the one or two paragraph statement for district the assembly. This report will be submitted to the General Superintendent.	Must be signed by the Pastor
2010 Fair Share Verification*	This form is required to verify your 2009-10 Fair Share of your church's weekly tithes and offerings based on the financial policies of the Central Florida District.	Must be signed by the Pastor
2010 Local Church Directory Page*	This form is used to report local church contact, pastoral and lay leadership information for the 2010 District Jounral. If you do not submit a new form, we will use information from the 2009 Journal.	Information to be verfified by the district
